How To Create A Quiz Or Survey With Convertful
With Convertful you can create multiple steps quizzes or surveys with different possible outcomes for your website. You can then integrate Convertful with a CRM like Platformly and send the results automatically to your CRM, using segments and tags to filter out the results.
For example, let’s say we have a business that offers dog and cat training classes and related products for sale. This business is also in the process of planning to bring in other pets related products for sale in the near future. We could create a quiz using Convertful to capture leads and send them to Platformly separated out as follows:
- Dog owners that are interested in training classes.
- Dog owners that are interested in dog related products.
- Dog owners that are interested in both.
- Cat owners that are interested in training classes.
- Cat owners that are interested in cat related products.
- Cat owners that are interested in both.
- Dog and cat owners that are interested in training classes.
- Dog and cat owners that are interested in dog and cat related products.
- Dog and cat owners that are interested in both.
- Other pet owners with indications of what pet they own, so you can see if there is enough interest for you to start selling products related to these other pets.
Sounds complicated, right? Well, here’s how you can easily do it using Convertful and Platformly.
- Connect Convertful to Platformly (or another CRM). You can read more about how to do this here.
- Create the segments and/or tags that you want for each
different group of contacts in your CRM. For our example, we will
create the following:
• Segments: “Dog Owners”, "Cat Owners” and “Other Pets”.
• Tags: “Dog Training class and events”, “Cat Training class and events”, “Dog Products” and “Cat Products”. - In Convertful, select the site and click on “Create
Widget”.
- You will then be taken to our template directory. You can
select one of our pop-up templates under the “Segmentation Survey
Pop-Ups” section or select “Blank Canvas” if you would like to
create one from scratch.
- For this example, we will show you how to create one from
scratch.
- Once you select “Create Pop-up”, you will be taken to
the builder where you can edit the widget as required. You can
rename the widget at the top left section of the editor.
- Each tab in the builder represents a screen that will be
displayed to your visitors. For a survey or quiz, the first
screen is where you want to put the question with different
answers that your visitors will need to pick to segment them out
into the groups that you want. In our example, we have the
question “Which of the below best describes you?”
- The next step is to add the possible answers for your
visitors to choose from. You can add images, text, buttons and
much more by selecting “Elements” from the side toolbar and
simply drag and drop the image, text, button, etc. to where you
want it to be shown. For the possible answers, you will need to
add buttons to the widget. You can read more about how to
customize your widget and all the different elements here.
You can add images if you want as well but note that images do take longer for the widget to load and display to your visitors.
- Then add the screens that you want to be displayed to the
visitors after they make their selection. To add a new screen,
click on the + button at the top of the widget tab and select
“Blank Screen”.
- You can add your next question or a sign up form here.
Remember to rename each screen to make it easier for you to set
up the next step of the widget.
For our example, we will make the next screen a second question for dog owners and name it “Option 1”, with the question “What are you interested in hearing more about?” and the answer, “Dog Training Class and Events”, “Dog Products” and “Both”. We will also create Option 2 for cat owners and Option 3 for those that own both dog and cat.

Option 3 - Create the next screen that you want to be displayed and so on. For this example, as we have given the visitors three possible answers, the next screens after Option 1, will be three corresponding sign up forms, Option 1A, Option 1B and Option 1C.
- To add a form to your widget, select “Form Fields” from the
“Elements” section. For our example, we have kept it simple,
asking only for the first name and email address and have
inserted the checkbox for agreement to our Terms and Privacy
Policy.

- We will use the same image and text for all three dog owners’
sign up forms, so you can simply duplicate the screen by hovering
over the right corner of the tab and you will see a 3-dot icon,
which if you click on will give you the option to rename,
duplicate or delete the screen.
- Once you have all your screens created, the next
important step is to go to each screen and set up what
will happen each time a visitor clicks on one of the buttons you
have created. To do this, click on the button and the “Button
Settings” will appear. Click on the “On Submit Action”
button.
- Click on “Add Action” and select what you want to happen when
a user clicks on the button. For our example, if a user clicks on
the “Dog Owner” button, we will select “Go to a widget
screen”.
- We then get a list of all the screens that have been created
for this widget. Simply select the screen that you want to be
displayed next. In our example, when a visitor clicks on the “Dog
Owner” button, we will select the on submit action as “Go to
widget screen Option 1”, which will show them the second question
we have lined up for dog owners.
- So for Option 1, we would set up the on submit action as
follows:
• If a visitor clicks on the first button “Dog Training classes and events”, they will go to the widget screen “Option 1A”.
• If a visitor clicks on the “Dog Products” button, they will go to the widget screen “Option 1B”.
• If a visitor clicks on the “Both” button, they will go to the widget screen “Option 1C”.
- Then go to the widget screens for Option 1A, 1B and 1C and
set up the on submit action for the “Sign Up” button. Note you
can add more than one action, so in our example, we will select
the following on submit action for Option 1A, who are dog owners
and are interested in dog training classes and events:
• Go to the Widget Screen “Thank you”.
• Add user to the Platformly segment “Dog Owners”.
• Add a new tag “Dog Training class and events” to Platformly user.
- If you do not see the segment or tag that you want in the
list of actions, hover over the right hand side of the text box
and refresh the connection with Platformly and all segments or
tags that you have created in Platformly in the beginning will
appear.
- Repeat the process for cat owners and people that own both
dogs and cats. For example Option 3C is for those visitors who
are dog and cat owners, who are interested in both dog and cat
training classes and events as well as related products. As you
can see in the below screenshot, you would add seven actions for
the “On Submit Action”, being:
• Go to the Widget Screen “Thank you”.
• Add user to Platformly segment “Dog Owners”.
• Add user to Platformly segment “Cat Owners”.
• Add a new tag “Dog Training class and events” to Platformly user.
• Add a new tag “Dog Products” to Platformly user.
• Add a new tag “Cat Training class and events” to Platformly user.
• Add a new tag “Cat Products” to Platformly user.
- For other pet owners, we will create an Option 4 screen,
which is just a sign up form like the others, but with an extra
custom field for the owner to fill in what other pet they
own.
- Then for the on submit action, you would select:
• Go to the Widget Screen “Thank you”.
• Add user to Platformly segment “Other Pets”. - Remember that you must create a custom field for “Other Pets”
in Platformly (or other CRM) as well and be sure to map it
accordingly in your Convertful / Platformly integration
settings.
- Then set up your Display Rule for the widget and toggle on
the “Activate” button to make your widget live. You can read more
